Community Referral:

Please send signed physician prescription/referral, including:

• “Speech Therapy Evaluation and Treatment”

• Specific diagnosis, such as “failure to thrive", "prematurity", "speech-language delay”

• Patient contact information; name, DOB, address, and best contact number

• Insurance information

Hospital/Clinic Referral:

• Takes2Therapy (all one word) and Carmen Huston are listed in AllScripts and Athena systems

• Please call Carmen Huston at 512.865.9845 or email carmen.huston@takes2therapy.com with any questions

Once the referral is received:

• Family will be contacted to assure that they would like to proceed with therapy 

• Takes2Therapy will contact the insurance for care notification and pre-authorization as needed 

• Family will also be encouraged to call insurance to be fully informed of their benefits

• Appointment will be made to see the patient usually within one week of receiving referral
